The Montevideo CBOC is seeking a Board Certified Primary Care Provider (MD or DO) to join the Patient Aligned Care Team (PACT). The provider will deliver comprehensive outpatient primary care services to Veterans, focusing on a personalized, holistic, and coordinated approach to health care.
$155 - $160 an hour
Work Schedule

Hours: Monday through Friday, 8:00 a.m. – 4:30 p.m.
Status: Full-Time (1.0 FTE), defined as 80 hours every two weeks
On-Call: No on-call duties mentioned; coverage is for scheduled clinic hours.

Key Responsibilities

Direct Patient Care (Approx. 80% of time)
Comprehensive Care: Provide diagnosis, evaluation, treatment, and care management for acute and chronic conditions (e.g., hypertension, diabetes, hyperlipidemia) across various settings, including mental health, geriatrics, and gynecology.
Preventative Services: Conduct annual physicals, screenings, and vaccinations to maintain patient health and prevent disease.
Acute Care: Treat common illnesses and minor injuries such as colds, flu, rashes, and sprains.
Health & Wellness: Provide counseling and patient education on nutrition, weight management, and healthy lifestyle choices.
Care Coordination: Initiate referrals for specialists, social work, and homeless veteran programs. Consult with referring physicians to ensure appropriate clinical work-ups.
Medication Management: Prescribe and manage medications according to established policies.
Administrative Duties (Approx. 20% of time)
Documentation: Maintain timely and accurate medical records in the VA Electronic Health Record (CPRS/EHR) system.
Test Results: Communicate test results to patients in a timely manner.
Quality Improvement: Participate in continuous quality improvement meetings and activities.
Discharge Education: Provide follow-up instructions coordinated with the next care setting.

Qualifications & Requirements

Licensure: Must hold a current, full, and unrestricted license to practice medicine (MD/DO) in any State, Territory, or Commonwealth of the United States or the District of Columbia.
Board Certification: Must be Board Certified in Internal Medicine (ABIM) or Family Medicine (ABFM).
Certifications:
Basic Life Support (BLS) required (AHA or equivalent).
Advanced Cardiac Life Support (ACLS) required.
Registration: Must possess a current Drug Enforcement Agency (DEA) registration and a National Provider Identifier (NPI).
Citizenship/Legal Status: Must comply with all provisions of the Immigration and Nationality Act; E-Verify compliance required.

Additional Details

Workload: Average of 12 patients per day with appointment times varying between 30 and 60 minutes.
Technology: Proficiency with Electronic Medical Records (EMR) is required (VA uses VISTA/CPRS)
