KeystoneCare is seeking a dedicated Physical Therapist to join our Home Health team in the Philadelphia area. This position offers an exciting opportunity to provide personalized rehabilitation services to patients in the comfort of their homes, ensuring they receive the best therapeutic care tailored to their individual needs.
The Physical Therapist will assess, plan, and execute treatment programs that focus on improving mobility, strength, and overall functional abilities in patients of all ages. Collaborating closely with a multi-disciplinary team, you will work to enhance the quality of life for our patients, guiding them through their recovery journey with compassion and professionalism.

- Graduate of an accredited Physical Therapy program
- Current Pennsylvania Physical Therapy License
- Minimum two years of experience in Home Health or similar settings
- Valid Driver’s License and reliable vehicle
- Proof of Auto Insurance
- Strong communication skills for effective collaboration with patients, families, and the care team
- Excellent organizational abilities and capacity to manage a diverse caseload
- Proof of COVID-19 Vaccination or an approved Medical or Religious exemption
- Proof of Influenza (Flu) Vaccination or an approved Medical or Religious exemption
Physical Therapy: 2 years (Required)
License/Certification:
PA Physical Therapy License (Required)
