Luna Physical Therapy is seeking 2-3 licensed Physical Therapists to provide outpatient care directly in patients’ homes in the Jacksonville region. This mobile model allows therapists to treat patients 1:1 while maintaining their own schedules. Treat as little as 1-2 patients/week in your chosen specialties within 30 minute radius.

Responsibilities
- Provide outpatient physical therapy in patients’ homes
- Develop and implement individualized treatment plans

- Communicate effectively with patients and care teams
Requirements
- Active Florida Physical Therapist license
- BLS certification
- Personal liability insurance
- Reliable transportation
$65 - $72 an hour
